Regarding direct team-members (residents, interns, attendings), students reported that specific, individually-directed feedback is infrequent. It was further reported that student notes on patients are typically signed by interns/residents who do not read them and/or simply do not provide verbal or written comments.

Is this a problem, and if so how gravid? Do students fail to solicit it or do instructors fail to provide it? Should there be scheduled "feedback time" or should students simply ask more often?

As suggested by the "defining roles" thread, resident-student time for 15 minutes before rounds may provide an opportunity for feedback on progress notes.

I agree - specific feedback was rare. Also, we are required to turn in copies of H and Ps to the clerkship director, who does not work on the wards. This is a great opportunity for feedback on written write-ups that is unfortunately not taken advantage of. I did not get any comments back on these write ups until after my time on the wards was over, and the comments that I did get were minimal.

Specific "feedback time" exits between students and the clerkship director, but this is more aimed at the clerkship in general, not the student's specific performance. These weekly meetings are very important and should be continued, but it might be helpful to integrate some amount of performance feedback into them.
